The easiest way to access the Qualifying Presentation Evaluation form (Item 204) form is to go directly to the Church Speaker  Library page on theConnection.

The Speaker Library is accessed through theConnection. Sign in to theConnection > Select RESOURCES > Media/Resource > Media Resource Home > Speaker Library > Church Speaker. This will present you with the Church Speaker Library page. This page contains a Resources area that includes the Qualifying Presentation Evaluation form.

Alternatively, you could access the Speaker Media page of the Speaker Library, select "Document" as the Media Type, and search for "Qualifying Presentation Evaluation."\

Finally, you can access the global search feature on theConnection by selecting the magnifying glass, entering 'Qualifying Presentation Evaluation 204' in the search text field, and making the 'Document' box checked.
